Transferring to or Returning to Midway? All forms can be downloaded below for your convenience or you may pick them up in the school office.

Follow the instructions below...

Returning to Midway... 1. Complete both sides of the Student Information (ID) Form and return to child's homeroom teacher. All changes must be updated on the form.

 

Transferring from another school (in-county or out of county/state)...1. sign a Transcript Request for Records Form --2. Make a copy of the child's social security card--3. Make a copy of the child's birth certificate--4. Make a copy of the child's most recent shot record--5. complete a Student Information (ID) Form--6. Bring all copies and forms to the Midway office

 

 

***Please note...Court Issues such as custody, protection orders, restricted pick-ups, etc. MUST be handled through the office. Please make sure the office has copies of any paper work for your child's record.
